In short:
A Werribee factory is facing charges after a worker died when a metal bucket fell from an electrical hoist. WorkSafe alleges the company failed to maintain safe systems of work.
A concrete pole manufacturer was charged after a worker died at a Werribee factory in September 2024.
The 36-year-old worker died after a metal bucket detached from an electrical chain hoist and fell about 15 metres, striking him.
WorkSafe charged Vertech Hume Pty Ltd with two counts under section 21(1) of the Occupational Health and Safety Act, alleging the company failed to provide, so far as reasonably practicable, a safe working environment.

It is also alleged the company twice breached section 21(2)(a) of the Act by failing to provide or maintain safe plant and safe systems of work to prevent the bucket from dislodging while the hoist was being operated.
The matter was listed for a filing hearing at Melbourne Magistrates’ Court for 16 December 2025.
